Explain the importance of a substance use disorder counselor having an understanding of tolerance and withdrawal. What potential issues could arise if the counselor did not have this knowledge? Provide at least one example in your response. Additionally, discuss how a substance use disorder could mimic and/or co-occur with a variety of neurological, medical, and psychological disorders as discussed in the Differential Diagnoses sections of the Substance Use Disorders chapter in the DSM V
It is important for a counselor to understand tolerance and withdrawal because it is a factor that is considered when diagnosing, making a treatment plan, and detecting a substance use disorder (Capuzzi & Stauffer, 2016, pg. 152). However, it is not necessary that tolerance and withdrawal be a part of the diagnosis, but past withdrawal symptoms is associated with more severe clinical course (American Psychiatric Association, 2013, pg. 484). Not being educated on either of these concepts can lead to life threatening treatment plan, an ill-conceived treatment plan, under educated clients on what to expect at withdrawal, but also the client would be unaware of serious medical conditions due to tolerance from a substance. For example an individual who started using at a younger age means that more of the substance has been used over their life time and there will be more side effects or substance-related problems to be aware of when planning their treatment. If a counselor is not educated on tolerance and withdrawal he will not be able to treat the patient and moresoever he will not able to get the insight of the patient .
Its advised that consultation with specialists in infectious diseases, cardiology, pulmonary medicine, hematology, neurology, and surgery may be warranted. Whenever possible, consent should be sought to involve the patient's primary healthcare provider in the coordination of care. Co-occurring medical conditions present an opportunity to engage patients so medical staff should be alert . By focusing on the adverse effects of the substance abuse on the overall health of patients, staff members are in a position to help patients see the importance of engaging in treatment for their substance use disorders. Patients should have appointments for followup care made prior to detoxification discharge for all chronic medical conditions, conditions needing further evaluation, and substance abuse treatment.
