A 62-year-old woman with diabetes has an average blood
pressure of 162/104 mmHg. You recommend for her initial treatment:
Explain
a. Avalide®
b. Cozaar® (losartan)
c.Tenormin (atenolol)
d. Lozol® (indapamide)
e. Calan® (verapamil)
e) Calan (Verapamil) is the best medication recommended for hypertensive patient along with diabetes.
Reason-it has shown to decrease fasting plasma glucose level in diabetic patients. Studies shown that Type 1 and 2 diabetes mellitus on insulin with or without oral drugs, who received oral Verapamil has decreased fasting plasma glucose level than who did not use Verapamil. Also use of oral Verapamil is associated with a lower incidence of type 2 diabetes mellitus when compared to other calcium channel blockers.
Reasons for not using other drugs
a)avalide -it's not be used with any other drug containing aliskiren (person is having diabetic)-rare condition it can cause kidney failure. It can only be used if the person is having diabetic nephropathy caused by type 2 diabetes mellitus.
b) Cozaar (losartan) -losartan appears to help kidneys of patients with type 2 diabetes mellitus only who do not have high blood pressure.
c) Tenormin( atenolol)-beta blockers increases blood glucose level, which contributes to new cases of diabetes. Hence it is not preferred for those patients having diabetes as it can increase blood glucose concentration.
d) Lozol (indapamide)-can be used only in those patients having type 2 diabetes mellitus. It has very little effect blood glucose control, hills people with diabetes mellitus need additional insulin keep the blood glucose level steady. High blood sugar may occur for people with the risk factors for developing diabetes mellitus.
