You are caring for a 62-year-old woman who has had type 2 diabetes for more than 20 years. She is blind and has renal failure. Her 28-year-old daughter asks you what her own chances are for developing diabetes. The daughter reports that her 38-year-old sister has just been diagnosed with type 2 diabetes. You note that the daughter is 5' 4" tall and weighs about 220 lbs. She is a secretary and has a 1-year-old son.

1. What personal questions should you ask the daughter to help determine her risk?

2. Explain the genetics of type 2 diabetes.

3. Does the daughter have any risks for other health problems? If so, what are they?

4. What suggestions do you have for the daughter to reduce her risk or delay the development of the disease?

A patient with diabetes is admitted to the hospital for treatment of an infection in his foot. He wishes to continue living alone after discharge from the hospital. His current insulin regimen includes 6 units of rapid-acting insulin before each meal and 35 units of long-acting insulin at bedtime.

1. How would you assess this patient's ability to accurately and safely administer insulin?

2. You assess that the patient has difficulty drawing solution into a syringe but can administer insulin accurately. What skills should you evaluate to determine if the patient can safely administer insulin using in a preloaded syringe?

Expert Solution

A 28 yr old woman asking her risk of developing diabetes due to genetic factors.

She is 5 feet 4 inches tall and her weight is 220lbs. She is a secretary and has a 1 yr old son.

1. as a nurse I will ask her the following questions to determine the risk of diabetes.

  • dietary pattern(what kind of diet she is taking? how many calories she is taking per day?)
  • lifestyle(how much time daily she spent in her exercise? how much time she actively involved in doing physical activities?)

2. There is no specific genes or genetic pattern responsible for type 2 diabetes but there is somewhat increased risk. Genetic factor alone plays a major role in the development of diabetes, it is always associated with lifestyle and dietary pattern.

Here the daughter has an increased risk of diabetes because she is overweight and leaving an inactive life along with her genetic factor is positive(Her mother is having diabetes).

3.

Yer, the daughter has in the risk of many chronic non-communicable diseases.

- Heart disease (She is overweight, and live an inactive lifestyle)

- Stroke

- Hypertension

4.

Suggestion nurse should give to reduce her risk or delay the development of the disease include:-

  • Reduce your weight
  • Do daily Excercise
  • Live an active lifestyle
  • Limit carbohydrate in the diet
  • Avoid saturated and trans fat
  • Regular follow up and health examination
