The two areas in healthcare management that present ethical dilemmas are :-

1) Patient confidentiality : It is one of the ethical issues faced by the healthcare providers when they reports in the best interest of patients. This reporting may be in conflict with patient confidentiality. Patient confidentiality helps to recognize and protect the patient's right to privacy. There is obligation to preserve the patient confidentiality because :-

  • It maintains the integrity of the doctor - patient relationship.
  • It helps in avoiding the discrimination in the healthcare services.
  • It respects the patient's right to privacy.

Fulfilling this obligation is, however, complex. There are some situations in which healthcare providers are forced to violate the patient confidentiality. Some reasons for this violation are :-

  • In the situation where disclosure of the patient's information is in the interest of whole community.
  • Obligatory disclosure may sometimes be required by a court.
  • In the situation where sharing of patient's information among other healthcare professionals is required.

  • Paediatric and elderly patients may not have decision making capacity.
  • They may not understand the significance of their medical conditions.
  • In some situations, they cannot give informed consent to the healthcare providers.
  • In some situations, they are not able to select one choice over another.
