Reflect on a patient who is beyond 20 weeks gestation and presented with a health problem that commonly arises during pregnancy. Describe the patient’s personal and medical history, drug therapy and treatments, and follow-up care. Then, explain the implications of the patient’s health problem
Common problem arise after 20week of gestation is repeated miscarriage mainly due to chromosomal abnormality of foetus, cervical incompetence. Other few causes of miscarriage at 20weeks of gestation are eclampsia, antiphospholipid syndrome, lupus etc. So after 20weeks it is mandatory to usg and maternal hormonal assay. If there is history of previous child with any chomosomal abnormality like down syndrome then check acetyle cholienesterase, amniocentesis, cordocentesis, chorionic villous sampling. If patient has high blood pressure pedal oedema in morning and abnormal weight gain then all this feature indicate preeclampsia then take antihypertensive drug and other treatment according to severity with consultation of doctor.
Take tetanus doses according to schedule. Take iron folic acid daily. Check weight monthly or during antenatal check up... Weight gain approximately 2kg per month during pregnancy. Check foetal movement by mother. During ANC check fetal heart sound as it is surest sign of living foetus. Taje proper balanced diet and calorie requirement 300kcal more than normal state during pregnancy. Do some meditation as in this time some patient have psychical depression
