Legal requirements for reporting workplace health & safety incidents: -
- The Occupational safety & health act (OSHA) was established by United states and it requires compulsory reporting to the authority within eight hours of death of any employee related to work or hospitalisation of three or more employees. Ex: Death of any worker must be reported in eight hours to the authority.
- Any employer who receives form from Bureau of labour statistics must submit its annual injury to employees and illness related to work.

The purpose and value of incident reporting are as follows: -
i) Incident reporting help management to recognise and identify potential problems that needs proper attention and monitoring.
ii) It provides management with data of past incidents that had occured.
iii) A prompt reporting results in immediate plan of action enacted to ensure such incident does not arise in the future.
iv)Such prompt reprorting can save management from huge time loss payment of injured workers from grave incidents in the future.

Organisation importance of performing incidence investigation are as follows: -
- Finding out the exact cause why the incident occured. Ex - In case of fire in factory finding out why the fire broke out.
- Finding out the probability of reoccurance of the incident. Ex - Finding out whether such fire again break out in the future.
- Enumerating risk management controls in place to avoid potential reoccurance. Ex - Setting the fire alarm in place to control risk of life loss in fire.
- Giving stregth in the mind of workers that it is a safe place to work. Ex - Ensures the workers are not scared.

The steps for incident investigation process are as follows: -
i) Immediate action - The investigation must begin very soon after the incident happened. Ex: Preserving scene, collecting cctv footage.
ii) Investigation planning - This ensures proper planning to guide during investigation Ex: List of required resources, persons involved.
iii)Data Collection - This involves collecting data from all sources. Ex: Collecting information from Witness or cctv footage
iv) Data analysis - Analysing all the parts in a chain of events to arrive at potential pieces of conclusion.
v) Corrective action - This step involves avoiding raising connection only with direct causes of investigation, but also look after indirect causes which may be the potential cause for the incident.
vi) Reporting - This is the last step of the process and the report of entire investigation is published during the phase. Ex - Handing over the investigation report.
