Testing to confirm: What are the CD markers and cytogenetic abnormalities? Any relevant cytochemical stains?
Consider a mock case with Thrombocythemia
A 50 year-old woman comes in for a complete blood count (CBC) as part of a routine physical exam.
The results are:
WBC 7.5 K/µL
RBC 4.22 M/µL
Lym 28.7 %
HGB 12.4 g/dL
MID 10.4 %
HCT 38.6 %
Gran 60.9 %
MCV 91.4 fL
MCH 29.3 pg
PLT >>>> K/µL
MCHC 32.0 g/dL
RDW 13.5 %
The platelet count is = 1,068 K/µL (normal is 150-400 K/µL).
MID cells may include less frequently occurring and rare cells correlating to monocytes, eosinophils, basophils, blasts, and other precursor white cells.
Significant peripheral smear findings
Giant, bizzare platelets, platelet clumps, megakaryocytes or their fragment may be observed.Giant plateletes, leucocytosis,Erythrocytosis or slight anemia also present.
CD marker Janus activating kinase 2 (JAK2) V617F is a common mutation in Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs), which include polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF).
The most common Cytogenetic anomalies were trisomy 9 ,abnormal chromosome1 and Trisomy 8.
