The surgeon orders for wet-to-dry sterile saline dressing twice a day with iodoform gauze to the wound, covered with the wet-to-dry dressing. Explain how to perform this dressing change?
steps to perform this dressing are-
1- Check the patient's chart to make sure that wet to dry dressing is what the doctor requested.
2- Gather all material like sterile gloves,sterile solution, paper tape or micropore,4 by 4 gauze.
3- Wash the hands from fingertips to elbow then put pair of gloves.
4- Make the patient comfortable and explain about this procedure to reduce their anxiety.
5- Begin the process of removing dressing from edges of old dressing by using sterile saline solution to loosen the dressing.
6- Thoroughly clean the wound using saline and 4 by 4 gauze and piece of gauze use to pat the wound until dry.
7- Carefully inspect the old dressing and note down the odor, color and consistency.
8- Before apply new dressing, assess the patient's wound very carefully and note down any sign that wound is not healing properly.
9- Use a sterile solution to moisten few pieces of clean 4 by 4 dressing. Place the moistened gauze directly on patient's wound and follow with dry dressing.
10- Secure the dressing with paper tape or micropore. Evaluate the security of dressing and apply tape and guaze as needed.
