Gina, a 44-year-old mother of eight children, visited her physician complaining of a “bearing down” sensation in her pelvis, low backache, and urinary incontinence. A vaginal examination showed that the external os of her cervix was just inside the vaginal orifice and her perineum exhibited large keloids(masses of scar tissue). Her history revealed that she was a member of a commune located in the nearby mountains that shunned hospital births (if at all possible).What do you think Gina’s problem is and what caused it? (try to be anatomically specific)
The patient chief complaints including bearing down feeling of
the pelvis, low backache and urinary incontinence.
Patient also gives history of multiple child births which were all
delivered through vagina.
Uterus is a female reproductive organ situated in the pelvis
region. It consists of fundus, body, cervix and cervical canal. The
uterus is held at its anatomical position through its ligaments.
During child birth stretching of the uterine ligaments causes them
to weaken. The ligaments can no longer support the
uterus and causes uterus to descent from pelvis into the vagina. It
is common in women with multiple child births through vaginal
delivey. Symptoms include bearing down feeling in pelvis, urinary
incontinence, pelvic and back pain.
So on basis of signs and symptoms of the patient and owing to
multiple child births delivered through vagina, we can conclude
that Gina has conditiom known as prolapsed uterus.
