PCMH refers to Public Centered Medical Home. It is an effort to revolutionize primary healthcare to address chronic diseases and also to provide preventive care. PCMH is designed to build a healthcare system that is Patient-centric. A healthcare system that is responsive and respectful to the patient's needs, preferences, and values. The main reason to implement PCMH is to eliminate the Failures and poor health service provided to the patients under the current health system by providing comprehensive and high-quality medical care.
PHN or Public Health Nurse comprises of the biggest workforce in the public health and medical care industry. We play a vital role in the prevention of diseases and promote health and safety. While some care for a single patient at a time, PHN are required to look after an entire population. While looking after whole communities, we are also able to educate people about health, safety, and how to access healthcare.
We as public health services are deployed in government agencies, non-profit organizations, and also Public Centered medical homes. The newly designed and implemented public centered medical home service is a very suitable way of work for us. Our line of work is very public centered and PCMH are implemented just to do that. We are fulfilling various jobs while also providing healthcare advice to communities such as managing budgets, planning informative activities, and evaluating the effectiveness of healthcare programs.
