A patient was diagnosed with syphilis when RPR and TPHA turned out to be serologically reactive. After completing the antibiotic regimen, the patient’s serum was retested. RPR showed a non-reactive result, while TPHA showed strongly reactive. - Explain why the serologic test results came out as such following antibiotic treatment. (2 points). - What kinds of antibodies are detected by RPR and TPHA (2 points)
Ans1=RPR test indicates recent infection and is also useful in follow-up to monitor response of therapy. While treponemal tests(TPHA) are more specific tests, due to the limitation that they remain positive lifelong, they are not indicative of recent infection.so in this case RPR test is non-reactive after antibiotics treatment because recent infection treated. And TPHA test life long reactive it show past infection.
Ans2=RPR test is varient of VDRL test
RPR test: Results are available within 5 min
Rpr 2 types-
First -RPR card test. second -Automated RPR test
Uses cardiolipin Ag with carbon Immunoassay technique using latex
Antibody detected by this test is=
Particle to detect regain Ab Particles coated with lecithin and cardiolipin
TPHA=
Treponema pallidum hemagglutination assay
Microhemagglutination assay for IgM and IgG antibodies in which sensitized sheep erythrocytes are coated with T. pallidum (Nichol's strain)
