What local anesthesia and pain management is given to a 32-years-old man with compound fractured hand?
WALANT - Wide awake local anaesthesia no tourniquet techniques
WALANT are used currently for hand surgeries in which patient is wide awake and in this technique tourniquet is not used which is relieve for patient as didn't have to bear pain from tourniquet as it is unpleasant and unnecessary.Two medications as local anesthesia can be given in WALANT And can easily replace tourniquet use are epinephrine and lidocaine .
So in this cade WALANT can be used with local anaesthesia of epinephrine or lidocaine
PAIN MANAGEMENT -
Assessment of level of pain by pain scale
Immobilise the part immediately to reduce pain from bony fragments ( compound fracture or open fracture)by splint
Elevate the part to reduce edema of hand but don't place pillow under hand
Hot or cold compress given to reduce pain
Provide medications analgesics such as acetaminophen or ibuprofen and in combined to reduce pain in patients
