A physician has submitted a prescription for a catheter, however, they did not submit sufficient information on the prescription to process the order. How would you go about finding the missing information and correcting the order? Please describe your process in triaging these situations, including any research you would do to solve these issues, resources you would want access to, the scripts you may use in interfacing with these patients, and your plan and cadence of outreach. How would you go about ensuring that the same problem doesn’t happen again?

As long as catheter available on differnt sizes we need a clear order from doctor ,so that we can process for order and get it from pharmacy to start procedure.if the order are inaccurate to process we will inform the doctor politely that the order doesn't mention such and such information could you please add the remaining order so we can continue with processing of items.in this case usually if doctor by mistake has done he will rectify and we can can continue after made correction.

Some cases doctor ask us to prepare usual adu li t size or Male or female size in such cases if stocks are available in our department we can consider all the common sizes for the men or female .but if need to get it from the store it wont be possible so that dr need to correct the order with all the details.if he is not ready to do that we should inform the matter supervisor and rise incident report to avoid such things in the future.
