The problems in the patient care -

In the health care sector there are a lot of tools to handle the patients related data and to monitor the patients. If we talk about the care of the patients then the the traditionjal way is not widely used today. Today in health care system everything is specialized and the maqjor problem can be virtualized so that multiple experts can work on the problem at the sdame time. We discuss here some problems in the patient care system.

1) Unavailability of physicians - In the hospitals a patients in the treatment needs an expert doctor as well as a physician. So this is required to engage a physician with the patient to treat better and efficiently.

2) Unavailability of nursing staff - In hospitals it is required to keep sufficient number of nursing staff so that every patient can get better care everytime. Nursing staff cares the patients after the doctor and runs the treatement advised by the doctor.

3) Communication with patient - The nursing staff and the doctor as well should introduce himself to the patient to make a healthy communication with the patient. So patient can feel confident during the treatment. This is importent in the patient care. Some time there may ber a language barrier with the patients so this can be resolved.

4) The nursing staff or any other should not constantly interrupt the patient. The care should be taken by the nursing staff to monitor the patient and should not let anyone to interrupt them.

5) Always listen the patient concerns that what they need and what are they asking. If there is any change in medication which can caise littli bit of side effect then it should notified to the patients. And explain them what they need to know.

These are some general issues and the problems in the patient care process. The traditional process is not used but the current process of patient care with automated tools is widely used and it helps in the better patient care.

Accuracy concerns in data reporting -

The accuracy is very important concern of the data reporting in the healthcare system. Whether the data is internally or externally reported from sources the should be accurate. This will increase the quality of data.There are generally some problems or issues persists whemn data is reported. Here we discuss the general issues in the data reporting and the some steps to handle these problems and to increase the data quality.

The problems in the data reporting -

1) Interpretation or coding errors :-

In this type of error if the two abstracters looking for the a data element in a patient's medical record but extracting different data from the same source. So there will be variations in the dat interpretation and coding of the data from the same source and same data.

2) Data entry, transfer, or transformation error :-

When data is not entered accurately in the registry or data store then it is called data entry error. These errors decreases the data reliability and the quality.

3) Errors of intention :-

These types of errors are executed intentionally in the registry. This can be loose the consistency of the data so quality is decreased.

Steps for assuring data quality :-

1) Training :- Train properly the data collector people that how should data collected in which form and accurately.

2) Data completeness :- Data should be always complete. An incomplete data is errornous to the system.

3) Data consistency :- Should check the data consistency over the period of time..

4) Onsite audits :- Review audit should be conducted to improve the data collection techniques and the data entry. This will assure the data accuracy and increase the data quality.

So these are some steps which are essential to take to increase the data quality by asuuring the data accuracy and increasing data reliability.
