- medical records if stored in a laptop or any other electronic device without any security password , it is a violation. Because if it gets stolen any one can easily get the information. So it better if we keep a security password

- If medical records are stored in paper form , it is necessary to Store all these files in a locked room in night . Avoid to keep them in areas where anyone can reach easily

- A nurse can access private health information of any patient in her unit , but she is not allowed to get health information of patient of any other unit

- Hospital guards can get information regarding patient room number so that they can tell this to any family member of patient. But they are not allowed to get information regarding diagnosis and treatment of patient

- Releasing information regarding children without consent of their parents is violation

- Releasing information to a wrong patient is a violation. This sometimes happen when both patients have similar names
