Ms. Yao age 27, developed a DVT in her left thigh after surgery to repair a fractured right femur. She is receiving IV heparin and will transition to warfarin starting tomorrow.
1. Describe the pathophysiology of DVT. How does Ms. Yao's diagnosis relate to development of DVT?
2. Identify essential information you need to safety administer the medications prescribed
3. Develop a teaching for Ms. Yao
1) Pathophysiology of DVT:
•Venous valves are avascular, which, in conjunction with reduced drift of oxygenated blood in veins, predisposes the endothelium to be hypoxemic.
•The principal component of these venous thrombi is fibrin red blood cells, which get trapped in the clot.
•The skeletal muscle pump helps forestall DVT by means of shifting blood past the valves , which washes away activated clotting elements that can in any other case propagate the initial thrombus.
•96% arise in the lower extremities; 4% arise in the upper extremities.
•90% of PEs occur from DVTs.
•50% of symptomatic proximal lower-extremity DVTs have asymptomatic PEs.
•70% of PEs have a symptomatic DVTs.
•28% of symptomatic DVTs will have post-thrombotic syndrome after 5 years.
•Clot extension and embolization.
DEVELOPMENT OF DVT IN MS. YAO'S CASE:
•due to surgery.
•due to bed rest, the leg will not move then it may occur.
•due to venous stasis.
2) SAFETY MEASURES WHILE ADMINISTERING DRUG:
•first check that is there any reaction of that medicine on her or
not.
•regularly check the values of PT-INR ,PT, aPTT.
•any other medicine she was taking before or not.
•check platelets regularly.etc
3) TEACHING FOR MS.YAO:
•meet doctor regularly.
•regular checking for blood clotting profile.
•tell about drug interactions to Ms.Yao
don't take any other medicine without doctor's prescription or
permission.
•if bleeding, heart and lungs problem, loss in platelet count,
alopecia and osteoporosis occurs then immediately meet to
doctor.
