A hypothetical situation in which the worker has been supporting the client: a situation in which the client is terminally ill due to a chronic disease such as cancer. In this situation the worker is trying to meeting the needs of the client by establishing efficient communication between the client and the worker to find the preferences of the client based on the culture and religion of the client to deliver patient centered evidence based practice which is relevant in the clinical context. The worker also works together to identify and requirements for change to deliver the evidence based practice i.e. to ensure that new medications based on evidence are administered which have fewer side effects. The worker can monitor his/her own work to ensure the required standard of support is maintained by reviewing the patient outcome in terms of recovery, rate of mortality, rate of readmission, duration of stay in hospital etc. .
