A problem statement is a statement where one or two sentences
that identifies and briefly explains a situation, problem, or issue
that a quality improvement team may be asking to address. The
health problem is defined as the gap that occurs between an
acceptable or desirable health status and the current status. After
getting included in a Project Charter,a problem statement provides
a quality improvement team with an fluent expression of what they
are setting to achieve.Although, there is need to understand
difference between the symptoms of a problem and an actual problem.
Symptoms are used to know that there is a problem. They are also
the shadows of underlying problems,that is an evidence by which

problem affects staff and makes itself known. But in other terms
symptom is not a problem, but the
outcome of an actual problem. A problem is defined as the gap
between the existing state and the
