For the Integumentary System subsection of the Surgery section of the book, CPT classifies repair of wounds by simple, intermediate, or complex. What are the definitions for each of these types of repairs, and how are they different from one another? What are some other CPT guidelines related to coding for wound repairs?
Simple repair is used when the wound is superficial and involves epidermis or dermis or subcutaneous tissues and no involvement of deeper structures.It requires simple one layer closure .
Intermediate repair is the repair of wounds and involves epidermis,dermis ,deeper layers of subcutaneous tissue and non muscle fascia.
Complex repair is repairs of wounds requiring more than layered closure like scar revision, debridement ,stents , extensive undermining or retention sutures.
Guidelines for coding wound repair:
When multiple wounds are repaired add together the lengths of those in the same classification and from all anatomic sites are grouped together into the same code.For example add together the lengths of the intermediate repair to trunk and extremities .
Do not add length of repairs from different grouping of anatomic sites like face and extremities.Do not add together the lengths of different classification like simple repair and intermediate repairs .
When more than one classification of wound is repaired code the most complicated wound repair as primary procedure.Code less complicated wound repair as secondary procedure .Use modifier 59 along with code.
