A 23-year-old male patient is admitted with a fracture of C6 and C7 that has resulted in quadriplegia. He was injured during a football game at the university where he is currently a senior. His career as a quarterback had been very promising. At the time of the injury, contract negotiations were in progress with a leading professional football team.
1.Use Roy’s criteria to identify focal and contextual stimuli for each of the four adaptive modes.
2.Consider what adaptations would be necessary in each of the following four adaptive modes: (1) physiological, (2) self-concept, (3) interdependence, and (4) role function.
3.Create a nursing intervention for each of the adaptive modes to promote adaptation.
Answer to 1st question:
According to Roy's criteria, the adaptaion involves the environmental changes in case of health to which responding positively. The adaptive process involves the focal stimuli in which internal or external factors challenges the situation of person like poor health, poor diet , growth of that person. in male patient , focal ftimuli is fracture injury during foor ball game.
the contextual stimuli involves the situation influenced by the other stimuli like survival of person goals and socialisation. and contextual stiuli is his growth of life is promising and creats stress.
Answer to 2nd question:
The adaptations necessary in the four adaptive modes:
--The psychological mode involves the person's physical activites and mentally adapt to the changes like injury during game. in that situation, the patient should have proper amount of oxygenation , food has good nutrients, activity of person to that situation and has to take adequate rest for body to regain energy . The maintanance of body fluids and acid base balance is necessary within the body.
--self concept mode in adaptive process includes the person has to improve the view of his self in better way even he injured in foot ball game and adapt to develop belief in his self and maintain equilibrium in the mentality of that individual.
--Interdependence mode in adaptive process includes developing of better relationship of that person and his group of foot ball team with the increasing confidence to achieve and raise belief and respect of each other in group
--role function mode in adaptive process has understanding and behaviour of other person in various positions and maintain social integrity.
Answer to 3rd question:
The nursing intervention for this adaptive modes are :
psychological --the patient should take rest and has good nutrition to recovery from the fractures during football game. The nurse should teach patient to do daily activities and had to take more fluids to maintain body integrity,
self concept mode--the nurse should encourage patient to not lose hope on his future and about football game and patient may talk openly to nurse helps in avoiding of stress and develops self motivation.
interdependence mode --nurse and patient should have belief and respect to each other so that patient can feel comfortable .the patient should gain trust from the other group members and then the group members also provide strength to patient to recovery from injury. So nurse also teach patient exercises to relieve stress and trauma.
role function mode--the nurse should understand patient from his position and has patience to listen to his concerns and has to help him in the recovery.
