A 42 year old male patient in ICU, 3 days s/p MVI with head injury, currently with respiratory failure, on vent support and induced coma. Patient has active bowel sounds, and the following order is in the patient's chart: Initiate Tube feeding: Jevity, start at 20 cc/hr, advance by 20 cc Q4h, as tolerated, until goal rate of 80cc/hr. It's your shift. It's currently 9 AM. Describe step by step, how you would initiate the tube feeding, what you will be looking out for, how you would be advancing the TF, etc.

TUBE FEADING .

1. Assess the condition and base line vitals of the client to ensure to inetiate feeding .

​​​​​​2. Arrange all the articles near to the patient.

3. Drape a towel around the neck and chest.

4.check bowel sounds for any stomach upset or air or fluid.

5.aspirate gastric content to ensure the placement of the tube

6.flush the tube with some plane water.

7.pinch the tube at the end of the syringe and hold it little high and above the patients's head level.

8.pour the warm feeding 3/4 of the syringe and remove the pinch and allow the feed to drain down.

9. We need to refill it before the earlier portion completely drained down.

10.when the feed become 20 ml , give some water along with it and flush the tubings

11. hold and pinch the tubings to remove the syringe without any logding of air bubbles.

12.close the nasogastric tube and fix it in the clients head.

13. Remove the drape and replace all the articles.

14. Record the procedure in the client's file with the correct time, and amount of the feed.
