1. PHENYTOIN 100 MG CAPSULE

Phenytoin is used to prevent and control seizures ( also called an anticonvulsant or antiepileptic drug ) . It works by reducing the spread of seizure activity in the brain .

Dose to be given is 100 mg ( 1 capsule )

PATIENT TEACHING ON HOW TO USE MEDICATION AT HOME

  • Swallow the capsules whole . Do not use the capsules if discolored .
  • Take the medication with food if stomach upset occurs .
  • Take the medication with a full glass of water ( 8 ounces or 240 milliliters ) .
  • Use this medication regularly in order to get the most benefit from it . Take all doses at the same time each day to keep the amount of medicine in the body at a constant level .
  • Do not stop taking this medication without consulting the doctor .Seizures may become worse when the drug is suddenly stopped . Your dose may need to be gradually decreased .
  • Inform doctor if your condition does not improve or worsens .
  • SAFETY ; This drug may make you dizzy or drowsy . Alcohol or marijuana can make you more dizzy or drowsy . Do not drive , use machinery , or do anything that needs alertness until you can do it safely . Limit alcohol beverages .
  • OVERDOSE ; If overdose happens and has serious symptoms such as passing out or trouble breathing , call a poison control center right away .
  • MISSED DOSE ; If you miss your once daily dose , take it as soon as you remember unless it is within 4 hours of the next dose . In that case , skip the missed dose  .Take the next dose at regular time .  If you do not remember until the next day , skip the missed dose . Check with your doctor if you miss for more than 2 days in a row . Do not double the dose to catch up .
  • STORAGE ; Store at room temperature away from light and moisture . Keep away from children .
  • SIDE EFFECTS ; Headache , nausea , vomiting , constipation , dizziness , drowsiness may occur . If any of these effects persists or worsen , tell your doctor .
  • HOW TO TAKE ; Advice the patient to take the 1 capsule ( 100 mg ) orally BID ( twice in a day ) with a full glass of water . Take each dose at the same time every day .

2. PREVACID 30 MG CAPSULE

Prevacid capsule ( lansoprazole ) is used to treat certain stomach and esophagus problems ( such as acid  reflux , ulcers ) . It works by decreasing the amount of acid your stomach makes . It relieves symptoms such as heartburn , difficulty swallowing , and persistent cough .

Dose to be given is 30 mg ( 1 capsule )

PATIENT TEACHING ON HOW TO USE MEDICATION AT HOME .

  • Advice the patient to take the medication by mouth once daily , before a meal . Do not increase the dose or take this drug more often than directed .
  • Do not crush or chew the capsules . Swallow the medication whole . If you have difficulty swallowing capsules ;

** PREVACID capsules can be opened and sprikled on applesauce , ensure pudding , cottage cheese , yogurt or strained pears .

** PREVACID capsules may also be emptied into a small volume of either apple juice , orange juice or tomato juice .

  • Use the medication regularly to get the most benefit from it . To help you remember , take it at the same time each day . Continue to take the medication for the prescribed length of treatment even if you are feeling better .
  • Tell your doctor if your condition persists or worsens .
  • Advice the patient to report any symptoms associated with acute interstitial nephritis , diarrhoea that does not improve , fractures of hip, wrist , spine etc .
  • MISSED DOSE ; If a dose is mised , take as soon as possible . Do not take 2 doses at one time to make up for a missed dose .
  • OVERDOSE ; In the event of overexposure , treatment should be symptomatic and supportive . If over exposure occurs , call pioson control center .
  • SIDE EFFECTS ; Diarrhea , abdominal pain , or headache may occur . If any of these effects persists or worsen , tell your doctor or pharmacist promptly .
  • HOW TO TAKE ; Advice the patient to take 1 capsule ( 30 mg ) at a time .

3 . PAXIL 10 MG TABLET

Paxil ( Paroxetine ) is used to treat depression , panic attacks , obsessive - compulsive disorder ( OCD ) , anxiety disorders , and post - traumatic stress disorder . It works by helping to restore the balance of a certain natural substance (serotonin) in the brain . Paroxetine is known as a ( SSRI ) selective serotonin reuptake inhibitors .

Dose to be given is 10 mg ( 1 tablet) at a time orally .

PATIENT TEACHING ON HOW TO USE MEDICATION AT HOME .

  • Take the medication by mouth with or without food as directed by the doctor . Taking  this medication with food may decrease nausea .
  • Do not increase the dose or use this drug more often or for longer than prescribed . Your condition will not improve any faster , and your risk of side effects will increase .
  • Take the medication regularly to get the most benefit from it .
  • Keep taking this medication even if you feel well . Do not stop taking this medication without consulting your doctor .
  • It may take upto several weeks before you get the full benefit of this drug .
  • Tell doctor if condition persists or worsens .
  • SAFETY ; This drug may make you dizzy or drowsy or blur your vision . Alcohol or marijuana ( cannabis ) can make you more dizzy or drowsy . Do not drive , use machinery , or do anything that needs alertness or clear vision until you can do it safely . Avoid alcoholic beverages .
  • OVERDOSE ; If overdose occurs call the poison control center . Symptoms of overdose may include ; irregular heartbeat , fainting , severe dizziness , seizures .
  • MISSED DOSE ; If you miss a dose , take it as soon as you remember . If it near the time of next dose , skip the missed dose . Take the next dose at the regular time . Do not double the dose to catch up .
  • STORAGE ; Store at room temperature away from light and moisture .
  • SIDE EFFECTS ; Nausea , drowsiness , dizziness , trouble sleeping , loss of appetite , weakness , dry mouth , sweating , blurred vision , and yawning may occur . If any of these effects persists or worsens tell the doctor or pharmacist promptly .
  • HOW TO TAKE ; Take 1 tablet at a time . qd ( quater in a day ) means 4 times in a day for next 7 days .
