In a study of many diseases like rabies and many other where fatality is very high or almost 100 percent, there , it's unethical to have a comparison group under these circumstances we may prefer to choose pre-post trials or a study where we take the a whole bunch of people as an experimental group and study the effect before and after giving the drug. If the new drug or treatment is beneficial, then all patients will be benefited.
