The biologic monitor system used weekly in a dental office to monitor the success of sterilization processes has come back positive, indicating sterilization failure in the instrument-processing area. What are common errors found in the instrument-processing area and what might be their cause?

Instrument processing areas are places in health care facility where the surgical instruments after use is cleaned and autoclaved on small and large scale.

Proper processing is required for effective sterilization of surgical instruments. Some of the common errors in processing area and their causes are :

  • Alteration in humidity and temperature control.
  • Improper use of personal protection equipment. Not covering the head fully may expose the contaminated hair .
  • Using contaminated shoe covers.
  • Not loading the instruments in mechanical washer according to the instructions.
  • Not cleaning the sterile processing unit everyday . It has to be cleaned everyday irrespective of its usage.
  • Not sterilising the brushes that are used for cleaning .
  • Not replacing the reusable brushes often.

These are some of the reasons that results in sterilisation process coming back negative. The reason can be improper knowledge of person handling the sterilization . Therefore, the person has to be trained well to prevent these errors.
