Construct a nursing care plan from the following scenario: Mark Hansen, a 45-year-old man, is admitted with a fractured femur. He is placed in skeletal traction. His nursing diagnoses are impaired physical mobility r/t immobilization by traction and self-care deficit, bathing, grooming, and toileting, r/t immobilization.
Nursing care plan:
- provide adequate privacy to the patient
-Assess the patient health knowledge and cultural preference in
care
- monitor the patient elimination pattern and provide bedpan,
urinals and adequate time for elimination
- check the patient vitals regularly
- teach the patient active and passive ROM exercise to increase the
blood flow to the leg
- maintain hygiene and make sure the bed linens are dry and smooth
to avoid infection and skin breakdown
-avoid contractures and foot drop by providing a handroll and
footboard.
-change the patient position little by little without giving
pressure to the fracture area
-assess the patient for self-care activity to promote
self-care
- encourage the patient to cough and deep breathing exercises to
prevent respiratory complications.
-keep the patient well hydrated by increasing fluid intake
-provide a high protein diet and fiber diet
- interprofessional communication with occupational therapists and
physical therapists helps patients to encourage activity.
- administer analgesics for pain as per doctor instructions it
helps to continue the self-care tasks.
- reposition the patient with pillow support
-stool softener prevent constipation
-reduce the traction pull and be sure ropes not obstructed.
- check the extremities for color changes, edema, signs of
infection and pain etc
- avoid depression by encouraging family and friends to visit the
patient frequently
- provide TV and other entertaining activities to avoids depression
and stress.
