A.Focused physical examination is the examination of a patient based on the presenting complaint.In this either only the particular system is involved or two systems.
In complete physical assessment,examination of complete head to toe is involved.That means a thorough assessment is performed.
B.The scale that is used to assess the level of unconsciousness and the consciousness is known as Glassgow coma scale.It has four criterias and each one carries points.The criterias and the scores are the following
1. Eye opening the maximum score is 4
If there is spontaneous eye opening-4
Opens eye to verbal commands_3
Opens eyes to pain - 2
No response -1
2.Motor response maximum has 6 points
Obeys commands- 6
Localises pain-5
Withdrawal from pain-4
Flexion response to pain-3
Extension response to pain-2
No motor response-1
3 Verbal response maximum has 5 points

Incomprehensible sound -2
