Identify four (4) specific high reliability principle and described how they might be applied to the practice of risk management of patient safety.
The concept of high reliability is attractive for healthcare, due to the complexity of operations and the risk of significant and even potentially catastrophic conserquences when failure in healthcare occur. sometimes people interpret high reliability as meaning effective standardization of healthcare processes. however, the principles of high reliability go beyond that; high reliabilty is best described as a condition of persistent mindfulness within a organization. High reliability organizations cultivate resilience by relentlessly prioritizing safety over other performance pressures. High reliabilty organizations use systems thinking to evaluate and design for safety, New threat to safety continously emerge, uncertainity is endemic and no two accidents arte exactly alike. So the high reliability organizations work to create an environment in which potential problems are anticipated, detected early and virtually always responded to prevent catastrophic consequences. This mindset is supported by five charcteristics ways of thinking
1. preoccuption with failure
2. reluctance to simplify
3. senstivity to operations
4. defence to expertise
5. commitment to resilience
It is important to recognize that standardization is neccesary but not sufficient for achieving resilient and reliable healthcare systems. its is an ongoing process or an organizational frame of mind not specific to any structure.
