POLST means Providers order for life sustaining treatment. It is a legal document. POLST helps healthcare provides to take decision regarding effective care of an individual in emergency period. Polst has many components namely, full treatment, limited treatment and comfort measures. Full treatment include Cardiopulmonary resusciattion. Limited treatment include Medication, IV fluids, and cardiac Montor. Comfort measures include are medication by any route, positioning, wound care, masseging, oxygen, suction and other comfort measures. After discussion with an individual and his family, obtain POLST willingnesss from him, his family, thereby, It serves as a portable form in medical crisis, saves time ,energy and fulfill patinet and family wishes

Catatrophic illness people who are seriously ill and lives on support to reduce their sufferings and improve their life longer. Individual preferences on quality of life after severe and chronic illness are resussciate, medication, food and fluid,comfort measures, intensive and non intentive treatment. catatrophic illness are heartattck, cancer, stroke, long standing diabetes mellitus.

End of life care means is care is being provided by health care providers when an individual loses his ability to survive and continue his life on support of advance medical support.

there are other form of obtaing wishes and orders from an affected individual and his family are advance directivies which are living ill, medical power of attorney,donot resusciate, directive to physician. Living ill is being written when an individual is in healthy conditions. An individual appoints a person to take decision on behalf of him and verify his wishes regarding medical care in POWER OF ATTORNEY. in Donot Resusciate, patient express wishes about not to resusciate in emergency period. directive physicians is a wriiten document regarding to die without any medical measures.

advance directives are wriiten for an individual who may be healthy and conscience. But an individual with serious illness and dying, the POLSTA form should be filled earlier by the health care provider. therefore, health care personnel can provide care easily and timily if one has POLSTA form
