SBAR report is used in the clinical setting to communication about the patient. Use the scenarios given to write an SBAR report.
1. Mrs. T is an 89-year-old woman that arrived in the emergency room by ambulance from her assisted living facility. She is a no-code and no allergies. She had a fall from her bed and has dementia. She is not complaining of pain since given morphine at 1800. The x-ray shows a hip break. Her skin is intact and she is receiving NS in her right forearm. Vital signs are stable and family has been notified by the assisted living that she is in the hospital. Surgery is a possibility for the morning, but the surgeon has not confirmed this yet. The day shift nurse needs to give report to the oncoming night shift nurse. Please use SBAR.
Situation:
I am nurse X
I am the concern nurse for Mrs T
I am concerned about Mrs T,89 yrs old female who arrived to emergency room by ambulance from her assisted living facility.
Background:
The patient came to the hospital because she had a fall from her bed with hip break.
The patient's physical or mental limitations are dementia and bed ridden.
The patient is on morphine 1800 , surgery possible for the morning and waiting for the surgeon's confirmation.
IV fluid NS onflow through her right forearm.
Assessment:
Pain score checked-no pain reported since morphine is given at 1800.
Vitals checked and recorded. Vitals are stable.
Physical examination done, her skin is intact
Mental status examination done, dementia present
Radiological examination done: hip fracture present
Request:
I would like to discuss the following possible actions-
Consulation with the surgeon.
Scheduling a meeting with bystanders if possible.
Additional blood examination prior to surgery if present
Arranging blood for surgery if present.
Consultation with pain and palliative department as morphine is onflow
