B.Medical history of the patient.

  • Patient has a history of hypertension from past 3 years
  • Diabetes treatment is under insulin therapy

D.Objective data

A 58 year old male admitted with diagnosis of hyperglycemia. His blood sugar is 1500 and the patient is admitted in ICU. He's is on an insulin gtt per protocol. The goal is to maintain the patient blood sugar at 70-200 per md order.

E.Subjective data

Patient explain that it's the fourth time he has admitted to the hospital in the past year for hyperglycaemia. However, he states he has never been in the ICU and says "it must be a serious condition now".

The patient's wife explains that patient does not manage his blood sugar levels at home very well and eats whatever he like and remain several days without checking his blood sugar levels.
