Electronic Health Record, EHR is a record where one can store ones health records and information in digital form.This helps to provide better health care and also it is easy to share the the information with one another.There are many terminologies used in the EHR. They are:

1.CDSS : Clincal Decision Support System is a evidence based practice in EHR which helps to take best decisions in patient care and helps one to avoid the medication error.Also helps to increase the health care assistants.

2.HIM : Health Information Management is a practice of acquiring traditional medical information for providing quality healthcare.

3.HITECH : Health Information Technology For Economic And Clinical Health Act (2009) was made to increase the adoption of EHR for increasing the quality of health care.This act was made because it has proved that the EHR can help to improve the quality as well as the efficiency of the patient care given.

4.HL7 : Health Level 7 was a group which was formed to exchange the information and records between the independent and the computer application.This helps in the evaluation of health services, management and clinical practices.

5.Clinical Analytics : The process of gathering and examining data for knowing more about the patient.

EMR also known as Electronic Medical Record is the records of the patient which is stored in a hospital where all the records of the patient is in a digital version of a paper chart.This record only contains the record of one hospital.The medical record of a patient from one provider.

EHR also known as Electronic Health Record is the record where the patient's record from various hospitals are stored together from various providers.This is stored digitally.This is owned by the patient itself.This information travels with the patient where ever he goes.

PHR also known as Personal Health Record is similar to EHR and EMR.But here the patient itself is storing the information.The patient itself will be able to track the health conditions.It helps the patient for a perfect home care.

So the main differences between the EMR,EHR and PHR is that EMR is limited to one person and cannot be traveled. But EHR contains all the records of the patient and can be traveled where ever one goes.But PHR contains the records which is recorded by the patient itself.The records in PHR can be shared with the providers for a second opinion and for the follow up care.
