PLEASE READ THE CASE STUDY AND ANSWER THE QUESTIONS TO THE BEST OF YOUR ABILITY:
Mr. Jackson is 86 years old. He is admitted to HUMC for congested heart failure. He also has a history of renal failure and goes for dialysis on Mondays, Wednesdays and Friday. He has an AV fistula in his right hand. He also has a history of diabetes, hypertension and depression.
He is allergic to shrimp. He is a nonsmoker. He lives with his wife and two sons.
He is on a renal, low sodium, low cholesterol diet.
He is positive for clostridium difficile.
He suffered a stroke while he was in the hospital and now has left sided weakness.
4. What concerns do you have about his fluid and electrolyte status? Explain and give rationales
Ans-
Mr.jackson was suffering from renal failure.in this case kidney fails to do their function.kidney unable to filter and doesn't Maintain the electrolytes balance which is major function of kidney.
Patient suffering from CHF And hypertension.in this case patient need Diet having less amount of sodium.daily need of sodium in diet is 2000mg/day.high intake of sodium leads to problem of heart failure and patient may die.so avoid this sodium rich diet can be avoided.also patient suffering from renal failure so unable to filter sodium.so low take food with low electrolytes.
Increase in cholesterol level leads to heart problem.to avoid this intake of cholesterol rich diet should be low.it is divides in triglycerides,HDL,LDL.
Patient have suffering from infection of clostridium difficile which leads to diarrhoea.where the diarrhoea is condition in which loss of electrolytes from body.in this condition patient have need to maintain electrolytes condition.this is cure by vanomycin.
Patient having suffering from left hand weakness.the condition is called as hemiplegia.also called as left hand side or left part of body is weak in this condition.so patient suffering from paralysis condition.
Thus, having electrolytes in the right concentrations (called electrolyte balance) is important in maintaining fluid balance among the compartments. The kidneys help maintain electrolyte concentrations by filtering electrolytes and water from blood, returning some to the blood, and excreting any excess into the urine.
From overall condition we understood that,Mr Jackson have need the equel level amount of electrolytes.beacuse of diarrhoea electrolytes are loss and and if he take more electrolytes rich diet then kidney is unable to filter it.
