Proper history taking , examinations and ancillary tests are essential to come into correct diagnosis.And the clinical diagnosis can be confirmed by pathology reports.Informed consent should be made from the patient for admissions and operation.Also advanced directives can be made if patient can't take decision at that time.

Patient is not allowed to carry any of his property while comming for surgery so it's detailes should be noted before surgery.Before surgery the surgeon should go through the patient's face sheet, anaesthesia reports to know the conditions of the patient and after surgery it's detailes should be documented in surgery reports for further reference.

After surgery the patient should be under nursing care till he recovers.Meanwhile the physician should take down his patient's progess report and finally based on physicians report and recovery room record discharge summary should be made out before the patient leaves the hospital.Hence all these should be followed for further reference in case of any issues like subsequent medical conditions of the patient, any legal issues like medical negligence etc.Hence proper documentations are necessary.
