1-The chronic conditions/illness which we can generally notice are cancer, heart diseases,diabetes,HIV/AIDS,chronic kideny disease etc..

Chronic condition/illnes are the disease which last for more than 3 months and may extend beyond one year.These conditions require continues medical care and can't be cured completely,predominant in adults

For example,Diabetes is the serious and common condition which may even become fatal overtime.Thus it should be prevented before and if with onset of diabetes one should continues medical care and regular exercise

2-The impact of identified chronic condition / illness on individual, family and community:

-on individual:the patient with chronic condition has low self-esteem,looses interest in daily activities,gets depressed mentally,faigue,pain,negative self-image,mood swings, anxiety attacks, stress,anger issues

-on family :family members of these patients also develop low self-esteem,emotional instability and their social performance would be low, financial crisis,dull work performances,get socially separated and mentally disturbed..

-on community :decreased community participation, awareness amon people, social distancing,people get more acquainted with these conditions,developing knowledge about management of the chronic conditions

3-The role of nurse in coping with chronic illness, management and prevention of complications

-The nurses play a vital role in management of chronic diseases,they can cope up with these patients by maintaining healthy relationship with patient,being more in touch with them,by gathering information about patient,patient's family their social and economic factors to get more involved.She should assess patient condition and develop her skills and strategies

-Management and prevention of complications:patient should be educated about his disease and its management and nurse consultation is a crucial one. As these conditions take time so there should be continues regular treatment and checkups to prevent further complications

Nurse should develop strategies to enhance the effectiveness of treatment by regulating diet and excercise of patients

Nurse must also educate patient family and always listen to what patient tells and ensure proper services are provided at home, hospital and community

Nurse should maintain good mental health of patients and form support groups.......
