Nursing SBAR for patient with alzheimer's
SBAR
S ( SITUATION):
Mr:X 82 YEAR old Men admitted in ER with irritability and aggressiveness, he was repaeating questions and asks over and over , he was unable to recall his family members ,
B: Background :
Mr: X had a history of severe head trauma, he is also an obese man ,he was under home medications for high BP and sleepless ness, he has a intermittent history of get lost in familiar places ,he is a healthy individual except this conditions
A( assessment):
Mr:X has a regular HR, high bp ,clear breath sounds, head exam, Blood glucose, Urine output monitoring, Took morning medicine and Depression screen remarkable for moderate depressive symptoms: her PHQ 9 score is 15, Negaitive for energy, sleep, and speking ,poor coordination.loss of bowel and bladder control,swallowing difficulty etc.
R Recommendation
Based on my assessment patient is high risk for complications
1.Follow treatment guidelines to manage high blood pressure,
2.participation in social activitiesmentally and socially stimulating activities
3. do not miss medications and follow up traetments
4.excercise regularly
