- The policies can be state based ie state negotiates and decides the price of health care delivery on behalf of the citizens or it can be federal state partnership where the responsibilities are jointly shared or it can be federal driven where the state merely acts as a carrier for implementation
-The model is governed by regulatory oversight and mandatory reporting
-It supports a tiered model for healthcare delivery
- Aimed at a competitive market to provide affordable healthcare

Attainment of Health equity :Equitable and affordable healthcare for ALL cannot be achieved as one size does not fit all. A certain proportion remain deprived of proper healthcare.
- defining roles, agenda ,setting up of priorities ,defining responsibilities and authorities for federal, state and local governments and reviewing the role of non state actors
- Lack of funds : results in underinvestments in certain sectors leaving them vulnerableto any emergency health outbreak.
