Medical errors

Medical errors can be defined as failure of planned action to be completed or use of adverse affcet ot care which may or may not harmful to the patient.

Most common medical errors are adverse effect of drug, misdiagnosis, restrained related injury, mistaken patient identities.

It is important that all healthcare workers be aware that medical errors not only subject patient to harm but also lead to medical malpractice ligitation.

Steps to prevent medical error :

• Physician need to verify their orders before submission.
• The pharmacist will check and verify dose of drug and provide guidance about timing of drug to be given.
• The nurse before administer drug should check the right dose,right drug ,right route and right patient.

B) Importance of manager in patient safety:

• For correcting errors that can have adverse patient outcome.
• For taking up the responsibility to evaluate and improvement in delivery of healthcare activities.
• To maintain record of medical errors and evaluate cause behind it.
• Withholding effective communication which improve teamwork which help in quality care and ensure patient safety.
• Having patient centred care by ensuring standard and guidelines and assessing and managing that everyone is following it.
• A manager perform regular organizational review which help in improving patient safety.
