Kotter’s 8 steps of change focus on introducing organizational change and on managing resistance to change while encouraging buy-in by the applicable constituents which in the case of health care organizations includes both medical and nonmedical staff and employees, the board of directors, patients, and so on. Focusing on a change that you would like to make in your organization, share the reason for the change and whether these 8 steps will be applicable or not.

Kotter's 8 steps of change model are :

  1. Create urgency.
  2. Create a guiding coalition.
  3. Develop a vision and strategy.
  4. Communicate the change vision.
  5. Empower broad based action.
  6. Generate short term wins.
  7. Build on the change.
  8. Anchor the change in organizational culture.

I want to focus on bringing the change in patient's attitude and adherence towards medications in patients suffering from psychiatric disorders in a non-proft organization. The patients are unable to adhere to the medications and their attitude towards medications influences their adherence behavoiur. Patient's adherence status influences treatment success. So, there is necessity for a change to improve patients adherence and attitude towards medications

1. Create urgency

I will bring awareness about the necessity on improving patients attitude towards medication by focusing and convincing the physicians, nursing staff and pharmacist by summarising the potential threats like treatment failure, increased medical expenditure on organization, increased patient burden etc. I will explain the possible ways of minimising these threats like using paramedical student volunteers to emphasize the importance of medication adherence and attitude towards medications.

2. Form a powerful coalition

I will identify efficient pharm.d student volunteers and ask them to extend their emotional support and committment. A professional training will be provided for selected candidates about medication adherence and attitude importance and influence on treatment success.

3.Create a vision for change.

I will develop a vision about how the change brings treatment success and success for organization. I will ask the student volunteers to interact with patients before the initiation of treatment and follow the patient for three successive time periods to know the effect of counseling on medication adherence and attitude of patients.

4. Communicate the vision

I will communicate my vision to the physicians, nursing staff and hospital director. I will demonstrate the extent of cooperation i need for achieving this change. I will communicate the benefits for them in a clear and transparent way.

5. Remove obstacles.

I will check for any resistance from the student volunteers, physicians and nursing staff. If i found any, then i will resolve them smoothly.

6. Create short term wins

I will reward student volunteers if they succeed to bring positie change in patients medication adherence and attitudes towards medications in atleast 2 patients per month.

7. Build on the change.

After reaching short term goals i will review student volunteers who failed to meet this short term goal. I will find out the problems, what needs improving. I will set goals to increase the number of patients who improve their medication adherence and attitude.
