1. Identified client problems in service are Vision and hearing impairment. Norma physiological function of vision and hearing capacity is decreasing with age . After age 60 years this problems get worse. Consequences due to vision and hearing impairment. *Risk of fall due to vision problem (Presbyopia) ,Old age vision problem. *Risk for accidents due to loss of or decreased vision and hearing problems. *Vision and hearing impairment make old age people as dumb ,it leads to reduce communication with loved ones. *Due to impaired vision and hearing is there risk of taking medication wrongly (When caregivers are not available. Accident during old age leads to many complications for both family and the patient. Its a burden to family members economically and time consuming because they can not able to perform their works in old age due to accident they need some ones time . Its not easy to recover from accidents if they have major injuries due to compromised immunity levels. So I selected this topic. If we provide client teaching about care of persons with vision and hearing impairment it helps to reduce accidents and reduce burden to the family indirectly.
