Marvis is a 29-year-old G1P0 in labor at 40 weeks of gestation. She was admitted at 0200 with a history of contractions since 2200 the night before. On admission, she denied spontaneous rupture of membranes or vaginal bleeding. She felt good fetal movement. Her vital signs and the fetal heart rate were normal. Her contractions were every 6 minutes and lasting 30 seconds. Her cervical examination was 2 cm/70%/−2 vertex. It is now 0600 and she has been reexamined. Her cervical examination is now 3 cm/90%/−2 vertex. Her contractions are now every 4 to 5 minutes lasting 60 seconds. She states they are still mild.
Marvis has stated that she wants a natural unmedicated birth. Marvis expresses disappointment in her progress. What can the nurse do to help her at this point in labor?
As at 0600 the patient's current condition is stating means Mervis contraction are every 4-5 min lasting 60 seconds. And the mother expresses her disappointment . Before that period at 0200 she was in latent phase......at 0600 she entered in active phase . And another sign in this phase mother feels themself anxious, painful, irritableand also the cervical examination justifies it.
NURSING HELP AT THE POINT OF LABOUR----
1)continuous labour support- give the mother continuous emotional support,advocacy , comfort. At this stage the presence of her family member or her parents is important.
2) can be given comfort by providing ice chips or lip balm.
3) use of hydrotherapy with warm water for relaxation .
4)position change -- tey to assist the patient with comfortable postion by which the patient is feeling easy.it has been seen now a days recumbent position is the most beneficial . Changing the position can add the benefits of gravity to easy labour and also shape of pelvis.
5) massage can be beneficial ans it will increase endorphins production.
6) for relaxation , nurse should recommend to breathing techniques ......teel the mother to have deep clean ,rhythmic breathing.
7) continuous monitoring on foetal heart rate also the vital signs of mother, urination frequently.
8) using of cool soaked clothes on the forehead , for better relaxation.
9)you should have to be sure that the patient should not be senseless at that time.
10) Distraction methods like music ,television or any funny thing can be used to distract the focus from upon the anxiety and pain.
