Case study:

AL is a 41-year-old female presenting with a Candida sp. infection. Posaconazole is considered as treatment option. AL currently takes omeprazole 20 mg daily for treatment of gastroesophageal disease. Evaluate the concerns with this treatment option.

Based on the case study, answer these questions:

1.List each drug-drug interaction with existing regimen and pharmacology of interaction

2. List the medical risks due to each drug interaction

3. List any potential dose adjustments I.e. dose should be increased/decreased

When a patient having omeprazole along with Posaconazole , omeprazole reduces the action of Posaconazole. Omeprazole is a proton pump inhibitor and it reduces stomach acid. While reducing the stomach acid,it reduces the absorption and blood levels of posaconazole and thus reduces it's effectiveness against fungal infections.

So, when a patient is advised by taking posaconazole they should tell his doctor about the medication which he is taking previously. Usually the omeprazole dose may decrease or stop the medication until patient complete posaconazole dose. Or doctor may advice an alternative drug too.

When a patient taking posaconazole her blood electrolyte levels,liver enzyme levels and complete blood count should be monitored periodically. The patient may have hypokalemia,thrombocytopenia, and hyperbilirubinemia . Also seum level of drug also should be monitored to know the effectiveness of posaconazole for the patient.
