List and describe the valid authorization for release of information
HIPAA (Health insurance portability & accountability act), 1996 requires doctors to obtain written authorization from patients to share information in their medical record for reasons other than treatment. The authorization should include the following details:
?1. Description of information: Authorization must include meaningful description of necessary information to be released for necessary purpose eg. Research purposes
2. Name of the individual: Person who is authorized to make the requested disclosure
3. Name of the recipient: To whom the information has to be disclosed
?4. Purpose of disclosure: Reason for disclosure of information
5. Expiration date: An expiration date or an expiration event that relates to the individual
6. Signature & date: Signature of the person authorized to make decision with date
?7. Right to revoke: The person authorizing has the right to revoke it any point of time
