1.) A physician changes the battery for a single chamber permanent pacemaker pulse generator inserted one year-ago and repairs (and reconnects) the existing transvenous lead. How should these services be reported?

3.) A physician performed a coronary artery bypass graft procedure involving two grafts – a venous graft to the right coronary artery and a separate venous graft to the left anterior descending artery. How should these services be reported?
1) 33218- Repair of single transvenous electrode for a single chamber, permanent pacemaker or single chamber pacing cardioverter-defibrillator (for atrial or ventricular single chamber repair of pacemaker electrode)with replacement of pulse generator.
2.) The harvesting of the vein is included in code 33510. Add -on code 33518 which describes two venous bypass grafts performed .
3) The saphenous vein grafts are re- ported with add-on code 33518 which describes the two venous bypass grafts that were performed
