1.) A physician changes the battery for a single chamber permanent pacemaker pulse generator inserted one year-ago and repairs (and reconnects) the existing transvenous lead. How should these services be reported?
2.) Immediately prior to performing a single coronary artery bypass graft procedure, the same physician harvested a vein from the left arm to be used for the graft. What add-on, if any, would be reported for the harvesting of the vein?
3.) A physician performed a coronary artery bypass graft procedure involving two grafts – a venous graft to the right coronary artery and a separate venous graft to the left anterior descending artery. How should these services be reported?
Permanent pacemaker are used by patients having various kinds of arrhythmia and other cardiac problem. Before removing the battery, the physician should inform the patient and caregivers the possible complications that can arise during the procedure and get an informed consent. The physician should also prepare the nurse and other medications, ventilator that can come in use during the procedure.
2.For coronary artery bypass surgery ,the one of the graft material used is vein graft. But the site of vein graft preferred is from great saphenous vein. We can also use vein graft from other sites. So physician can consider harvesting the graft from saphenous vein rather than from the upper arm.
3.When two grafts are using, for bypass surgery, the caregiver should be informed regarding the possible complications that can arise from this and the risks involved should be discussed .
