TRIAGE
When there are insufficient resource for medical care of everyone at once,evaluation and categorization of sick or wounded is done by triaging.It is usually done in mass casualty situations who needs emergency transportation to another hospital for care .it is used in emergency rooms and walk-in clinics .In disaster or in war triage consists of brief assessments of victims that allows a nurse to classify victims according to the severity f the injury,urgency of treatment,and place for tretment.
When a patient comes to an emergency department,the first step is the triage nurse takes a brief medical history of the complaint and check the vital signs.in case of a mass critical situation like a bomb explosion another system of triage is used known by the name START (SimpeTriage and Rapid Treatment).The categories are the patients with minoror less injuries ,the injured whocould be helped by immediate transportation and deceases.A commonnly used rating system in an emergency department is a three tier system that uses the categories of emergent,urgent and non-urgent.
The one which is used in casuality situations which involves a colour-coding scheme using red,yelow,green,white and black tags is called advanced triangle.Red tags are categorized for immediate treatment of patients,who have a chance of survival if immediate treatment is given.Yellow tags are for patients who need observation and whose condition is stable and not in immediate danger of death.Green tags are reserved for the walking wounded,white tags are for patients for whom doctors care is not required.Black tags for deceased.

Primary assessment
a. The purpose of primary assessment is to identify any client problem that poses an immediate or potential threat to life.
b Gathering of information primarily through objective data and on finding any abnormalities ,immediately initiates intervention.
c ABC airway ,breathing and circulation as a guide to in assessing clients needs and asseses a client who has sustained a traumatic injury for signs of a head injury
