Care plan for traction is as follows when assisting with a.m and p.m care we should emphasise on the patient to do as much possible for himself but with the limits of constraints of immobilisation, facilitate those tasks which he is not able to perform without help,assesment of pat and traction set up for changing the bed linen make sure that the linen is smooth and dry and also reposition the pillows for support do change pillow cases often,when assisting with bedpan or urinal give adequate time and maintain privacy of the patient keep toiletries at reach and call bell also, check for daily bowel movement , encourage patient to eat prescribed diet which is high in calcium , protein,iron and vitamins , ensure plenty fluid and roughage intake,emcourage breathing exercises coughing to prevent respiratory complications,activity exercise for non affected extremities, remove any factors that reduce the traction or alter the direction maintain ropes and pulleys weights must be suspended,check patient position help to sit up,encourage patient to use head trapeze instead of elbows to move in bed,checl extremities for colour, pallor , numbness,odema,skin eruption or press sore. Patient may need diversional activities to relive depression and boredom patient can read , watch television, visit of family members can combat depression and make recovery easy.

Care plan for patient in cast is as follows we need to provide cast care by supporting an exposed cast with our palms of our hands to prevent indentation, ensuring that stockinet is pulled over the rough edges of cast.elevation of extremity above the level of heart , covering and keeping uncoated area warm,expose fresh cast to circulate air until dry , exposure of fresh synthetic cast until it is completely dry avoid wetting of cast.encouage position change,provide analgesic promoting non pharmacological pain relief , monitor signs and symptoms of cast as syndrome,claustrophobic patient is at a risk of developing cast syndrome and acute anxiety,nursing care for compartment syndrome ,call health care provider immediately if symptoms or signs of neurovascular complications occur and if hot spot occur indicating to under cast infection,encourage isometric exercises for muscular strength, avoid and report cast breaks.avoiding insertion of sharp objects,teach skin care encourage safety precautions like avoiding wet floor,stairs,encourage mobility and active self care .
