1. Alteration in health(Diagnosis)            Septic shock,lowering of blood pressure,tachycardia,    decreasing urine output,alteration in mental staus. 2. Pathophysiology of related to clients problem: Urosepsis is sepsis caused by an infection of urogenital tract.This happens by release of pathogens and pthogenic product by infection that ae recognised by receptors called pattern recognition receptors.Infectious organism enter into blood stream lead to systemic symptoms.Resulting in increasing burden of inflammatory mediators secondary to endotoxinemia.

3. Health promotion and disease prevention. Immediately seek medical attention and follow doctors instruction. Drink enough water. Washing the hands before and after using the toilet,wearing cotton underwear. Urinate immediately when it is needed.

Assessment- 4 Risk factors:Risk factors are more commen in women and older adults and the risk factor is for . Person who have history of urinary condition. . catheter use. .Diabetes patient . Intake of drugs which cause immunosuppression. . The person above 60 years old. . HIV or AIDS   

5. Expected findings ; Frequent urgent to urinate,blood in the urine,foul smelling urine. Malaise,Pain on the lower side of your bac,nausea and vomiting,extreme fatigue,variation in  the heart rate,Low body temperature,rapid breathing. 6. Laboratory tests : Urine testing,Urea and electolyte- for the determination of renal function. Blood cultures,Coagulation screen -if surgical or radiolagical drainage of source of infection is necessary. 7. Diagnostic procedures ; Diagosis is done by a sample urine sample, And the diagnosis is done by looking for another source of infection that is causing sepsis by using chest X ray to see at the lungs,or blood culture to loo for bacteria in blood stream. . Computerised tomography(CT) which is used to scan the abdomen and kidney for the complete picture of kidney.Ultasound scan may also help to see urinry tract ro diagnose urosepsis.

8. Safety consideration:Patient with urosepsis who have systemic symptoms best managed in the intensive care settings. Drink enough water,medication interaction.

9. . Nursing care; provide isolation and monitor the patientas indicated. Limit the use of invasive devices or procedure as possible, maintain good hand hygiene. Monitor the urine output and vital signs. Wash the hands before and after each care activity. 10.    Therapeutic procedure- Treatment begin with antibiotics to fight the most likely infectious organisms. Intravenous fluid given to maintain blood volume and blood pressure support. Mechanical ventilation if needed and use sepsis therapy. 11. Medication- Medication by using antibiotics that help rid of the bacteria.other method is to remove the source of  infection is by surgery. 12. Client education-  Discuss about the disease and should maintain proper hygiene,drink enough water 13.    Interprofessional care-sepsis education and team collaboration are an integral part of identifying and treating patients with sepsis.An interprofessional collaborative approach implemented in other institution to combat the life threatening complication of sepsis. 14. Complication-possible complication are organ failure,kidney damage,septic shock,scar tissue in the urinary tract,pus collection near the kidneys.
