When a chart is removed from a file, the medical assistant should place a card inside the OUTGUIDE and place it in the spot in the file cabinet where the chart was.
The medical assistant has both clerical and clinical roles. The most important clerical function is the careful management of the medical records. The effective management of these files is vital to the efficient operation of the medical office.
A chart or file is a paper medical record in an outpatient facility, and which contains confidential clinical information about the patient's health and treatment along with billing and insurance information.
Out guides are made after filing materials for folders and are
plastic sheets with a tab and a pocket for index cards.
In order to remove a chart , write the date, name of the patient,
your initials, and where the chart can be found on the index card,
then place it inside the out guide, and put the out guide in the
spot where the chart was.
The indication of an out guide is that the chart has been removed
and where it can be found. It saves the place of a chart in use by
someone. Any one looking for the chart can easily identify where it
is.
In other words out guide is a filing tool used to track a patient’s chart removed from the storage area.
