1 Failure Mode and Effect Analysis is a systematic method of identifying and preventing product and process problems before they occur.it is mainly used to prevent tragedy and it makes system more robust and fault tolerant. The steps in FMEA are Define the topic,Assemble the team,Graphically describe the process,conduct the analysis and identify actions and outcome measures.

2 The goal of managing the care environment in any health care organization is to provide a safe ,functional,supportive and effective environment for patients ,staffs and visitors. The seven programs are safety management ,security management,hazardous materials and waste management,life safety management,medical equipment management,utilities management,and smoking policy management.

3 Both of these are used to asses hazards or risks that might affect a health care facility and surrounding community.Key components in risk assessment are

Calculating Probabilitygeneral risk which include flood plain,hidden risks not previously identified,calculating historical data and predictive data.

Calculating impacts-it includes human ,property and business

Calculating Preparedness includes plans ,resources and partnerships

The organization conducts a hazard vulnerability analysis to identify potential emergencies that could affect demand for the organization services or its ability to provide those services,the likelihood of the potential emergencies occuring and the consequences of those events.The organization communicates its needs and vulnerabilities to community emergency response agencies and identife the community capability to meet its needs.

4 The framework for safet measurement and monitoring includes

past harm -both psychological and physical measures.

Reliability- measures of behaviour and systems.

Sensitivity to operations-the information and capacity to monitor safety on an hourly or daily basis.

Anticipation and preparedness-the ability to anticipate,and be prepared for ,problems

Integration and learning-the ability to respond to ,and improve from ,safety information.

5 The tools used are Cognitive tests,physical ability tests,sample job tasks,medical-physical and mental,personal tests,criminal background and credit checks,drug testing,performance appraisal and language proficency
