1.Sepesis is an extreme response to an infection.When body get infected due to any cause ,to fight threat flood of chemicals released into blood steam.This causes widespread inflammation which ,over time ,can slow blood flow and damage body organs. Wound infection can leads to potentially complicated sepsis
Treatment for Sepsis from wound infection : *Need aseptic wound dressing to prevent feom late stages of sepsis. *Wound dose not be neglected. *Sepsis can cause temperature changes like hypothermia or hyper thermia.To treat hypothermia hot application is needed, To treat hyper thermia antipyretics needed. *Intravenous Antibiotics therapy is needed to reduce infection and prevent septic shock. *Sepsis cause hypotension ,it maintain normal blood pressure and cardiac function Intravenous fluids should be administered. *Administer Pain killers to reduce wound pain,Carticoseroids to reduce inflammation,Vasoactive medication to increase blood pressure. *Insulin may needed if blood sugar level increased.
2.Individual Cues indicated improving wound care : *Maintaining normal body temperature after continuous treatment and wound care . * Reduced wound drinag . * Showing symptoms of Wound healing ,dried wound patches.
3.Polypharmacy is the concurrent use of multiple medications by a patient. Polypharmacy is most harmful than most useful. There are many negative consequences associated with with poly pharmacy. Those syndromes And consequences are , *Increased health care cost due to high cost of druds ,use of multiple drugs. *Adverse drug events ,Use of multi drugs leads to adverse reactions. *Drug interaction. Probability of a drug drug interaction increased with number of medications increase. *Medication adherence . * Functional status: Due to multiple drug use function or Activities of daily living is diminished. *Cognitive impairment. *Increase risk of falls in older adults due to decrease stability. *Urinary incontinence may occurs due to affected kidney function.
Patient teaching:Education is needed for patient who are polypharmacy . *Advice not to use multiple drugs *If person using multiple drugs instruct them to observe for adverse reactions like urinary incontinence,reduce functions of daily living. *Advise not to use other medications with put doctor prescription. *Encourage them to take medications according to timing to reduce adverse drug reactions.and drug drug interaction.
